Application Procedures

Below are links to application forms you will need to apply to the dental hygiene program. If it is indicated that the form is in PDF format,  Acrobat Reader is required to view and print.  

If you are not presently a student at Chattanooga State, go to Online Application 

If you are already a student at Chattanooga State and wish to apply to an allied health program including dental hygiene, or if you wish to learn more about the Health Occupations Basic Entrance Test (HOBET), an entrance exam required for dental hygiene,  please go to the Allied Health website.
